Seven Hills RI is a human services agency that supports individuals with developmental disabilities. The case manager position works with the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals (BHDDH). We have a self-directed program through BHDDH funded services that enables individuals and their families/guardians to obtain services that best suit their needs.

Responsibilities:

• Process applications for the program
• Screen applicants for eligibility
• Monitor and track referrals
• Intake for new clients
• Track plans through the process of authorization with BHDDH
• Educate families/guardians on services available to clients
• Fiscal oversight of each person's budget
• Ongoing communication on programmatic changes and updates to the Self-Directed program

Qualifications:

• Bachelor's degree in Human Services or related field
• 2 years experience in related service area
• Bilingual (Spanish or Portuguese)

Job Type: Full-time

Pay: $25.00 per hour

Expected hours: 40 per week
